The film begins with , but Michael climbs into the river, where a Hermit keeps him healthy. About a year later, Michael reawakens, kills the hermit and returns to terrorize Haddonfield, where his long-lost niece,
Jamie Lloyd (played by
Danielle Harris ) lives. Jamie, who has nearly lost her ability to speak since attacking her foster mother, has developed a telepathic link with her evil uncle. Dr. Sam Loomis realizes that this link exists, and plans to use it to put an end to Michael. Michael begins stalking Rachel's (Jamie's foster sister) friend Tina (played by
Wendy Kaplan ), and Jamie lets them know where Myers is. Michael escapes the first time, but the second time he succeeds and kills Tina. Jamie agrees to help Loomis, and with Jamie's help, Loomis brings Michael back to the old Myers House. Michael makes many attempts to kill Jamie, and when he finally has the chance, Jamie says "Uncle." and Myers pauses and takes his mask off. Jamie sees his face, but soon Michael begins to attack, and he pulls his mask back on. Loomis comes to the rescue and attacks Michael, knocking Myers out. They take Michael to a sheriff's station. However, a stranger has come to Haddonfield. The film ends with an explosion at the sheriff's station, after which both Michael and Jamie have disappeared.

- According to the map of Haddonfield on the official website, halloweenmovies.com, the hermit in the prologue maybe another living relative of Michael Myers. Because of Myers' history, the hermit took up residence in the outskirts of town living alongside the Lost River.
- Michael Myers can be seen twice with his mask off- (1) In the hermit's cabin, while the hermit is in the foreground, Michael can be seen in the background for a second without his mask. (2) Toward the end, in the scene where Jamie is in the attic- it is a lot darker, but some of Myers' facial features can be made out.
- This Halloween installment introduced the Thorn tattoo and the mysterious stranger, known as the "Man in Black." Both are explained further in .
