Investigation of the wreckage revealed that the #3 engine and its pylon had separated from the airplane. During the separation, the #3 engine struck the #4 engine, which also separated from the airplane. A more detailed investigation revealed that the pylon midspar fittings, which attach the pylon to the lower portion of the wing front spar, had failed.

The airplane was manufacturer's serial number 22390, line number 482, registered as B-198, and was built in 1980.
