The eastern terminus of NH-119 is at the Massachusetts state line in New Ipswich. The road continues into Massachusetts and becomes MA-119 in the town of Ashburnham . The first 75 yards of NH-119 in New Hampshire, beginning at the state line, are in Hillsborough County and the town of New Ipswich before the road crosses into the town of Rindge and Cheshire County.

The highway runs nearly due west until it reaches the Connecticut River at Hinsdale. At Hinsdale the road takes a turn north and runs along the river for 5 miles (8.9 km). Across the river from Brattleboro the road again turn west and crosses the river into Vermont. Over the bridge in Brattleboro the road becomes VT-119 for about 400 feet (122 meters) until it ends at the intersection with US-5 . VT-119 is known as the shortest highway in Vermont.

Total length of NH-119 in New Hampshire is approximately 39.4 miles (63.4 km).
